They dont.

This is different by country, but to use the Premier League here in old blighty as an example.

League position: 1-4 = Champions League
5 = Europa League
FA Cup Winner = Europa League
League Cup Winner = Europa League

So top 4 in England at the end of the season get Champions League football. 5th and the cup winners go in to Europa League. Gets complicated when some of those positions are the same team (e.g. 2nd place may also win F.A Cup etc).

The Champions League has a group stage at the start. Top 2 teams in each group go through to knockout stages, 3rd places drop in to Europa League and 4th place go out altogether.

No. But if you won the CL and didnt finish in the qualifying places then you can defend your title in place of the 4th place team.
